If you require a sick note and this is your first request for the current illness, please be aware that you do not need to contact the GP surgery for the first 7 days of absence.
Employees are able to self-certify their sickness directly with their employer for the initial 7 calendar days. During this period, a GP-issued sick note is not required. Please see the easy download form attached > Self Certification Form - 7 days.pdf
If your illness continues beyond 7 days, you will need to arrange a consultation with a clinician. Following this assessment, a fit note may be issued where appropriate.
