Central Middlesbrough PCN will be providing appointments from Park Surgery Monday – Friday 6:30pm-8pm and Saturday 9am-5pm.

We will be offering a range of appointments with a wide range of clinicians such as GPs, ANPs, Physiotherapists, Pharmacists, Nurses and Healthcare Assistants. 

For non-urgent advice or treatment, send us your query via the E-consult banner below. Online Consultation requests can be made at any time and will be responded to as soon as possible, usually within 2 working days.

If possible please try to telephone before 11am if you require a home visit and one of our Patient Advisors will assist you. 

A doctor or nurse may phone you back if they feel that your problem can be dealt with by telephone advice.

House visits are only available for patients who are housebound because of illness or disability.

Please remember that several patients can be seen in the practice in the time that it takes to make one home visit. There are also better facilities for examination and treatment of patients at the Surgery.

It is important that you inform the reception staff if you are unable to attend your appointment, this will allow that appointment to be offered to another patient. If you fail to notify the Practice that you are unable to attend, you will be sent a letter informing you that you have defaulted from your appointment. Persistent defaulters maybe removed from the list.

You can also complete our appointment cancellation notification request form. This can only be used if your appointment has been arranged for more than 24 hours in advance. (excluding weekends and public holidays). You can also text to cancel your appointment.

We have a texting service which allows you to receive confirmation and reminders about your appointments.

To have this service you will need to register by completing a consent form.

Please remember to update your contact details with us when you change address, telephone numbers and email address.

Medical students and Nursing students attend the practice from time to time. We hope that you will co-operate with us and help the students to learn about general practice.

You will however be informed of their presence in advance and, if you do not want them to be present at a consultation, your wishes will be respected. This will not affect your treatment in any way.
